Ischemic colitis encompasses a range of syndromes arising from occlusive and nonocclusive vascular diseases.
Overall, the age-adjusted and sex-adjusted incidence rates of ischemic colitis in the general population were 16.3 per 100,000 persons. Individuals suffering from ischemic colitis may have symptoms including acute abdominal pain, hematochezia, and diarrhea. The broad overlap of symptoms including acute mesenteric ischemia, inflammatory bowel disease, or infectious bowel disease complicates the diagnostic process, and therapy may be delayed.

The development of ischemic colitis is based on two different mechanisms which make the pathology of ischemic colitis widely diverse. The first and most frequent mechanism is the reduction of blood flow, which results in an imbalance between the blood supply to the bowel and its metabolic requirement. This non-occlusive form of dysregulation is seen in individuals with cardiac failure or shock - whether it is hypovolemic, hemorrhagic, or septic in origin. Other causes of non-occlusive ischemia are colonic obstruction due to volvulus, colon cancer, fecal impaction, strangulated hernia, or pseudoobstruction. In these conditions, blood flow is reduced as a consequence of gut distension due to rising intraluminal pressure.
